Instructional Event # 5:

As has been indicated through the previous activities, transition planning is a coordinated set of activities leading toward determined outcomes for students.
Being educated about the available services, both in the secondary setting and adult world, will assist educators, students, and families in accessing the best services to meet an individual student’s needs.

Below are informational pieces regarding needs and services for students as they plan for transition. After you review each area, develop a list of who serves in that capacity or meets that need in your community or surrounding area. Follow up by requesting information from each resource including: who to contact, what services they provide and to whom, how to make application, etc… This information will not only benefit you, but all of those on your team involved in transition planning. Remember that one of our goals in transition planning is to support students in learning to become as independent as possible in their chosen communities. We must be connected to assist our students in meeting their goals!
