Prior to the meeting, the Coordinator should:
- Send the Notice of Evaluation to the parent(s)
- Determine who the make up of the committee
- the members will be individuals who have a knowledge of the student,
- the meaning of the evaluation data, and
- the meaning of the placement options
- Document the evaluation data
At the meeting, the Coordinator should:
- Gather data from variety of sources
- Consider the data obtained
- Determine if there is a need for additional data/formal assessment
- Determine eligibility
- Is there a physical or cognitive impairment?
- What is the major live activity impairment?
- Is there a substantial limitation?
- Is the disability too severe for 504?
- Determine if the student is eligible for a placement or accommodations
- Does the student need services because he/she qualifies for a disability?
- Determine and if needed identify in-class services?
- Determine if related services are needed?
- Determine if a Behavior Intervention Plan and/or a Functional Behavior Assessment are needed.
- Determine if other programs such as tutoring are needed.
After the Evaluation meeting the Coordinator should:
- Construct a report of the evaluation meeting for the parent(s)
- Copy the accommodation plan for the parent and student (if appropriate)

Prior to the meeting:
- Provide Notice of the Evaluation/meeting to the parent(s)
At the meeting:
- Determine the new evaluation date
- Determine if the student is still eligible for a 504
- Determine if there is a need for any change in services
After the meeting:
- Prepare a Report of Evaluation and meeting for parent(s)
- Prepare and copy the revised Accommodation Plan if appropriate

If the student is eligible, then a Section 504 Plan will be developed to give him/her access to the general education curriculum. Unlike the IEP for special education, there are no legal requirements for what should be included in the plan. A free appropriate public education (FAPE) under Section 504 often means identifying reasonable accommodations to help her succeed in the classroom. An accommodation plan usually addresses the following:
- Nature of the disability and major life activity it limits
- Basis for determining the disability
- Educational impact of the disability
- Necessary accommodations
- Placement in the least restrictive environment (LRE)

You will need to answer the following:
- Is this a proper referral?
- What problems does the student demonstrate?
- What are the student’s grades and ISTEP scores?
- Are there any disciplinary referrals?
- Are there signs of a disability?
- What prior informal attempts to accommodate for weaknesses have been made?
