MODULE DESCRIPTION:
The purpose of this module is to
introduce teacher candidates, practicing teachers, parents, community providers,
and other interested parties to Transition Services for students identified with
various disabilities. The intent of this module is to not only present what is
required by the legal mandates surrounding transition, but to identify issues
and practices that support successful movement for secondary students. The major
domains imbedded in transition planning are introduced, including: secondary
education, community access, independent living, vocational training,
employment, and adult services. At the completion of this module the
participants will have a broad understanding of the importance of transition
planning for students with disabilities as they move into secondary education
and on into the adult world.

INSTRUCTIONAL OUTCOMES:
Upon completion of this module the individual will be able
to:
- Understand the principles behind the
implementation of transition planning for students with disabilities.
- Locate and interpret the federal and state
guidelines and expectations surrounding transition planning for students with
disabilities.
- Identify the components necessary for successful
transition planning.
- Review an Individual Transition Plan and its
components.
- Address each of the identified transition
domains and indicate methods for reaching student outcomes.
- Locate resources for each transition domain,
with emphasis on community connections and adult agency support services.
